When you call Valley Connection, it works best if you have your Medical Record Number with you. You can call Valley Connection to:
- Make an appointment with a SCVMC primary care physician (PCP)
- Get a Medical Record Number
- Locate services
If you do not have a primary care physician, please contact your health plan to find our how to select your PCP.
If you have a Primary Care Provider with Santa Clara Valley Medical Center use Valley Connection to make or make or cancel an appointment.

- Enter your birth date using this format: MM/DD/YYYY
