A Medical Home is a partnership between a patient, their doctor and a team of health professionals. The physician leads the team in providing continuous and coordinated care throughout a patient's lifetime. The team supports the doctor in managing the patient’s care, and patients are involved in their own care and work with their team to reach their best health.
Healthcare services provided in a Medical Home include preventive services, treatment of acute and chronic illness, and help with end-of-life issues.
