Follow these instructions to submit your proof of refill and/or attestation form in the MedAssist portal.
- Click on the Apply Now button on the MedAssist homepage to enter the application portal. You can also visit the portal here
- Click on the “Returning Applicant” button
- Enter your Email Address and Password then click “Sign In”
- You will need to verify your log-in by sending a verification code to your Email Address. Click “Send verification code”
- Login to your Email in another browser. You will see an email from “Microsoft on behalf of SCCGOV-B2C”. Open the email to see the verification code. Copy this code and paste it in the Verification code box shown in red below then click "Verify code".
- Your Email address is now verified! Click on the Continue button to proceed to the portal
- Once you log-in to the MedAssist portal, you will see a summary of your grant award on the home page. Click on the My Grant button.
- You will see a breakdown of your monthly grant award amount. Click on Submit a claim to upload your proof of refill documents.
- Click on the type of prescription you are submitting a claim for. For example, insulin.
- Specify the start and end month and year that your proof of refill covers. For example:
If you filled an insulin prescription for a 30 days supply in April 2022, you should select April 2022 as your Start date and April 2022 as your End date.
If you filled an insulin prescription for a 90 days supply in April 2022, you should select April 2022 as your Start date and June 2022 as your End date. - Click on the Select a file button to upload your document.
Your proof of refill should have the following components at a minimum:
a. Pharmacy name
b. Pharmacy phone number
c. Pharmacy address
d. Medication name
e. Date Rx filled
f. Quantity -
You will see a green checkmark when your file is successfully uploaded. Click Submit Claim.
- You will receive a confirmation that your claim is submitted for review.
- Click on the Submitted Claim tab to see a history of your submitted claims.
- You can see if your claim has been reviewed under the Status column. You will see the claim that was just submitted in the red box below.
- Scroll down to the bottom of the My Grant page. The grant payment section will be selected by default. Here you will see all your paid and scheduled grant payments.
In this example, the patient received grant payments for February to April 2022. Their claim submission for May 2022 is scheduled. If you see a payment scheduled, that means your claim was approved. - Click on Details to see more about the payment that is scheduled for May 2022
- A pop-up window will appear showing details of the payment. The grant payment for May will be processed on or before 6/8.
- You can also click on Details for a previous month to see exactly which date your payment was issued. For example, for April 2022:
- A pop-up window will appear showing the date that the payment was made
Follow these steps to upload files for the MedAssist Application using an iOS (iPhone, iPad) or an android device.
UPLOADING FILES FROM AN IOS (IPHONE OR IPAD) DEVICE
- Click on the “+Add New Document” button and you will see the following options:
- Photo Library
- Take Photo or Video
- Choose File - You can upload files directly from your Photo Library, take a photo with your camera, or choose to upload a file that is stored on your iOS device.
- Select the type of document that you are uploading from the list that is shown. For example, Driver’s License
- Click “Upload this file” to start uploading your file
- You will see a green checkmark when your file is successfully uploaded to the portal
UPLOADING FILES FROM AN ANDROID DEVICE
- Click on the “+Add New Document” button.
- A popup will ask you to choose an action, please select one to upload your document:
- Camera: use this option to take a photo with your phone camera
- Camera/Camcorder: this option can also take a photo
- Files: use this option if you already have the photo/pdf file on your phone - Under “Document Type”, select the type of document that you are uploading from the list that is shown, for example: Driver's License
- Click “Upload this file” to start uploading your file.
- You will see a green checkmark once your file has successfully uploaded to the portal.
