The Valley Homeless Healthcare Program (VHHP) provides a number of services and programs at locations throughout Santa Clara County.Medical Respite
The Santa Clara County Medical Respite Program (MRP) is a patient centered, medically integrated program that serves homeless individuals that have been discharged as an inpatient from one of nine Santa Clara County Hospitals. This collaboration with the Hospital Council of Northern & Central California, HomeFirst Shelter Network, and VHHP provides medical, mental health and social services for referred clients. The MRP is a clean, safe place for patients to recuperate and continue with self-care rather than trying to recover on the streets.

Valley Homeless Healthcare Program operates a Medical Mobile Unit to provide medical care, social work and psychology services. Referrals are often made to Santa Clara Valley Medical Center for specialty follow up. Appointments are made on a drop-in basis. The following clinics are run out of the Medical Mobile Unit:
- SALUDOS: This mobile clinic serves migrant farmworkers in South County. The SALUDOS clinic is open from April through November, Monday evenings from 5-9pm.

- Medical Mobile Unit at the Encampments: This mobile clinic is held at the intersection of Story and Senter roads in San Jose and serves the homeless population living in encampments. The clinic is open on Tuesdays from 8:30-5:00pm. For questions call 408-885-3328.
- Re-Entry Medical Clinic: The Reentry Medical Clinic provides medical and psychiatric services to patients recently released from jail or prison. The clinic is located at the Santa Clara County Adult Reentry Center, which provides mental health, substance abuse, housing, employment, and financial services to the recently incarcerated clients. The Reentry Resource Center is open on a drop-in basis Monday - Friday 8:00am to 5:00pm.
The medical clinic is open Monday from 9-12pm, Wednesdays 1:00pm - 5:00pm and Fridays 8:30am - 5:00pm also on a drop-in basis. For questions, please call 408-535-4299.

Medical Mobile Unit at Gilroy: The Gilroy mobile clinic serves the homeless population in Gilroy and the southern part of Santa Clara County. Every Thursday the mobile unit is parked in front of Saint Mary’s Church from 1:00pm - 7:00pm. For questions, please contact 408-690-6180

The Teen Van provides health care services to youth attending schools, primarily Continuation Schools, throughout Santa Clara County. Current schools include: Boynton High School, Fremont Union High School Educational Options, New Valley HS, Broadway HS, Foothill HS, Escuela Popular, Mount Madonna HS, and County Office of Education schools (Snell, Sunol, EDGE, and Odyssey). Services provided focuses on prevention, and general health care to family planning.

The Backpack Homeless Healthcare Program (BHHP) provides health care services to the unsheltered homeless population in Santa Clara County. BHHP partners with other community homeless agencies to bring needed health care services to homeless persons who lack access to primary health care services. BHHP provides health care services on the streets, parks, and encampment sites, wherever patients are located. The team conducts medical outreach on Wednesdays from 1pm - 5pm.

Two dental vans provide dental care in several locations throughout the County. Santa Clara Valley Medical Center (SCVMC) has partnered with Ronald McDonald’s House charities to provide pediatric dental care five days a week for patients ages 20 and under. The Ronald McDonald van provides care at VHC at Sunnyvale and on the SCVMC campus.
