COVID-19 COMPREHENSIVE CARE (C3) CLINIC

Overview

To help ensure care continuity and provide COVID focused follow-up care, Santa Clara Valley Medical Center has opened its C3 clinic at Valley Health Center (VHC) Downtown, to provide outpatient care for COVID-19 patients who have been recently discharged from the hospital, or who have been seen at, or referred by, emergency rooms or primary care clinics.  

Clinic hours are Monday to Friday 8:30 AM to 5:00 PM except for Wednesday morning. 

Patients are followed through the acute phase of their illness, usually for 10-14 days after testing positive then discharged back to their primary care provider.  

This clinic has seen almost 300 patients since opening, and is actively managing close to 30 patients.
